From your local party store:

Fork and spoon
Balloons
Round party trays for the fruit and vegetable trays
Tiny cups for the dips/eyes
Party favor items

From the grocery store:

Various fruit (we used strawberries for his face, oranges for his mouth and blueberries for his mouth and eyes)
Fruit dip
Various veggies (we used tomatoes for his face, carrots for his nose and black olives for his mouth and eyes)
Vegetable dip
